Some services provided are not covered under our contract with the NHS and therefore attract charges. Examples include the following:
- Medicals for pre-employment, sports and driving requirements (HGV, PSV etc.)
- Insurance claim forms
- Passport signing
- Prescriptions for taking medication abroad
- Private sick notes
- Vaccination certificates
If you require any forms to be completed or signed you do not need to make an appointment with the doctor. Simply leave the form at reception and the staff will give this to the appropriate doctor. If necessary, the doctor will contact you to clarify any details or may request you to make an appointment.
The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
- Requests for Your Health Details
- Travel Insurance
More and more insurance companies are asking for your health information when booking holiday cover. If you have any doubt about the significance of a specific area of your medical history, check with your usual doctor at an appointment, before completing the form.
- Solicitors’ Letters
For PSV, HGV, DVLC, driving licenses, employment medicals etc will be done on request through reception – a charge is made for this service which will need to be paid for in cash.
Examinations relating to insurance policies are not carried out on our own patients.
